All athletes must provide their own health insurance to participate in athletics at Peru State College.  You will not be allowed to practice or compete until your insurance information is on file with the Athletic Trainer.  The college makes a policy available for purchase to those students without coverage.  Information about this policy can be obtained by e-mailing the Head Athletic Trainer at bmelvin@oakmail.peru.edu or at the website www.sas-mn.com.   Do not wait until you arrive on campus to obtain your health insurance.

New athletes print and complete all forms.  Mail forms to the address below or bring them with you to the start of your sports season.  Do not mail the informational forms.
